Medica HealthCare Plans offers special Case Management and Wellness programs for members of MedicareMax ChronicCare. If you would like to learn more information about these programs, please contact the Member Services Department Medica HealthCare Plans MedicareMax ChronicCare (HMO SNP) is a Medicare Special Needs Plan and is only available for Medicare beneficiaries with diabetes and certain cardiovascular disorders. There are no referrals required to see plan specialists with benefits that include a Point of Service benefit that allows for coverage outside of the network, comprehensive medical benefi with low or no copayments, unlimited transportation to plan approved locations. The cost sharing for covered Part D prescription drugs is $0 for Generic Tier drugs in Tier 1, $5 for Preferred Brand Tier Drugs in Tier 2, $20 for Non-Preferred Brand Tier Drugs in Tier 3, and a 25% coinsurance for Specialty Tier Drugs in Tier 4.
