ICD – 9 CODING TOOL:

The ICD – 9 Coding Tool is a reference tool designed to assist the health care professional or facility with the identification of those ICD – 9 codes that are in the CMS HCC Part C Risk Adjustment Model, and that should be reported via encounter or claim submission by the physician, if applicable to the patient.

Diagnoses reported from physicians are used in the CMS HCC Part C Risk Adjustment model. A pertinent patient diagnosis recorded by the physician can be reported repeatedly with no adverse consequence.

Documentation Guidelines

The Documentation Guidelines is a reference tool designed to assist physicians with the medical record documentation necessary to support the recording of CMS HCC Part C Risk Adjustment Model ICD-9 codes reported via encounter and / or claim submission to MHP.
